Hoje em dia um dos assuntos que quem está fora do peso mais discute é a cirurgia bariátrica. Em minha clínica tenho recebido vários pacientes que realizaram a cirurgia ou mesmo indico alguns para o procedimento.

Normalmente a pessoa acha que a cirurgia vai resolver todos os seus problemas e que o pós-operatório é simples e não vai ser necessário fazer dieta nem exercícios. Aí reside o principal fator de insucesso de algumas cirurgias.

Primeiramente o paciente que vai realizar a cirurgia deve ser bem selecionado e necessita fazer uma avaliação psicológica. O endocrinologista indica o processo cirúrgico se o mesmo apresentou falência de tratamento clínico, ou seja, não emagreceu pelos tratamentos convencionais.

Isto é importante porque recebo em minha clínica muitos pacientes que realizaram a cirurgia e são portadores de compulsão alimentar. Neste caso não há perda de peso, porque o paciente continua a comer sem fome, ou então ele muda o foco da compulsão, que pode se tornar: álcool, jogo, drogas ou sexo com evidente problema social.  O núcleo cerebral que coordena estas compulsões é praticamente o mesmo.

Outros fazem a operação e são pacientes deprimidos ou ansiosos, que comem por estes motivos e, se não estiverem fazendo acompanhamento destas doenças, podem piorar destes quadros, pois muitas vezes a comida é uma fuga.

Alguns nem chegam a tentar fazer dieta e já encaram a cirurgia como salvação de seus problemas de saúde, o que é temerário, pois intervenções simples poderiam evitar um procedimento cirúrgico.

Uma outra questão que frequentemente  é deixada de lado, é no caso do paciente gostar de ingerir alimentos moles ou de fácil deglutição como massas, doces, chocolates, refrigerantes e cervejas. Neste caso a cirurgia frequentemente não funcionará caso não haja uma mudança de hábito alimentar.

O principal fator de atenção no pós-operatório da cirurgia de obesidade é que o paciente deve continuar a realizar acompanhamento médico e nutricional. A depender do tipo de cirurgia realizada, o indivíduo precisa fazer reposição eterna de ferro e algumas vitaminas como as do complexo b e ácido fólico.

Em um artigo recente da Clínica Mayo (EUA), foi apontado que o índice de fraturas patológicas causado por cirurgia bariátrica tem aumentado muito nos Estados Unidos. Normalmente elas ocorrem 5 anos após a cirurgia. O estudo não condena a cirurgia, mas pede que os médicos estejam vigilantes em relação à prevenção da osteoporose que pode se instalar após a cirurgia.

Outra questão importante é que as mulheres que pretendem engravidar após a cirurgia devem aguardar cerca de um ano, um ano e meio para engravidar, até que a perda de peso se estabilize e não influa no crescimento fetal.

Em suma, mais importante do que perder peso, é analisar junto ao seu médico se você possui os pré-requisitos para fazer a cirurgia. A cirurgia tem alguns riscos imediatos de complicações, em uma taxa de 20%, e que podem aumentar muito se você tem problemas de coração, hipertensão pulmonar e propensão para trombose. A cirurgia bem indicada é muito eficaz e segura.  Não faça a operação por impulso, pense.

 

OBESITY SURGERY

Nowadays one of the subjects which are outside the most discussed weight issues is bariatric surgery. In my clinic, I have received several patients who underwent the surgery or even indicated some for the procedure.

Normally the person thinks the surgery will solve all the respective problems and that the post-operative care is simple and it won’t be necessary to diet or exercise. Therein lies the main factor of failure of some surgeries.

Firstly, the patient who will have the surgery must be well selected and needs to have a psychological evaluation made. The endocrinologist indicates the surgical process if the same presented clinical treatment failure, i.e. the patient did not slim down through conventional treatments.

This is important because I receive in my clinic many patients who underwent the surgery and suffer from a binge-eating disorder. In this case there is no weight loss since the patient continues to eat without hunger or else the focus of compulsion is changed, which can become: alcohol, gambling, drugs or sex with evident social problems. The brain core that coordinates these compulsions is virtually the same.

Others have the surgery made and are depressed or anxious patients who eat for these reasons and, if they were not monitoring these diseases, may worsen these pictures because often the food is an escape.

Some don’t even try dieting and regard the surgery as salvation of their health problems, what is foolhardy once simple interventions could avoid a surgical procedure.

Another issue that is often left out is the case of a patient who likes soft or easily digestible foods as pasta, pastries, chocolates, soft drinks and beer. In this case the surgery will often not work if there is no change of eating and drinking habits.

The main factor of post-operative care of obesity surgery is that the patient should continue to perform medical and nutritional monitoring. Depending on the type of performed surgery, the individual needs to make an eternal replacement of iron and some vitamins such as folic acid and the b complex.

In a recent article from the Mayo Clinic (USA), it was pointed out that the index of pathological fractures caused by bariatric surgery has increased a lot in the United States. Usually, they occur 5 years after surgery. The study does not condemn the surgery, but asks that the doctors are vigilant in relation to the prevention of osteoporosis that can ensue after surgery.

Another important issue is that women who plan to become pregnant after surgery should wait for about a year, a year and a half to get pregnant, until the weight loss stabilizes and doesn’t affect the fetal growth.

In short, more important than losing weight is to analyze with one’s doctor if the prerequisites for the surgery are given. The surgery has some immediate complication risks at a rate of 20% and that can greatly increase if you have heart problems, pulmonary hypertension and propensity for thrombosis. The well-indicated surgery is very effective and safe.  Don’t have the surgery done impulsively, think first.
